Performance Partnership Form
The evaluation form may be found below. The forms should be emailed to Employee Services by the deadline.
Form: Performance-Partnership-Form-Horizontal-Orientation
The HR website includes sections that should be reviewed by employees and by supervisor for guidance on preparing for the meeting, creating SMART goals, and resource information in FAQs and the competency library.
Training
Performance partnership training sessions are scheduled and listed on the HR website. More general information about performance partnership may also be found on the HR website.
